What distance should be between?

As much space as you can give. One of my apple trees. now has a spread of about 1/2 a meter and I only bought them 2 years ago.

How well do the trees do in winter? Can they be left outside?

Any tree will need protecting from frost in early months if it's first year would keep them by wall or fence to protect from cold and wind

Is it possible to swap out the pear tree for an extra gala apple?

unfortunately you cannot swap items from the bundle

What size pots should these trees be put in thanks..

As they are supplied in 9cm pots we would only recommend potting them on into a 15cm pot.
