Will this fit on a high cut ACH helmet without rails when using a headset?

Yes, the light clips onto the lower portion of your ACH (I guess you could call it the ear cover.) I still wear ear pro/comms under my ACH with this light clipped onto the lower portion of the helmet.

Would this fit on mlok rail? ex. hhv helmet and/or mtek?

No. But Tango Down sells a piece that allows it to attach it to a Picatinny rail.

What adaptor do i need for this to be mounted on arc rails?

Industrial Velcro holds mine on great. Amazon has the mount for $35 SureFire Ops Core Helmet Rail Mount for The HL1 Helmet Light
