Does this have the most recent update? And did they fix all of the earlier batch problems?

As Thomas said, you'll still have to plug the wheel into a PC to update the firmware, but all the problems have been fixed and works amazingly on the XB1 or PC :)

How is this wheel mounted? Example I have the Microsoft 360 Wheel & it has a clamp to attach it to a desk.

It clamps to the desk or table. Just like the 360 wheel.

I recently purchased and returned the TX 458, Just wondering if I should give it another chance?

Absolutely give it another try! I also had to return my original for a defect, but my replacement is doing fine. I just can't race Forza 5 without it. Racing with a controller makes no sense.
