Don't know terms - but can I mix iPhone headphone output with ukulele pickup output and output the result to headphones?

Not likely unless your uke can also directly drive headphones. Unless your ukulele has an internal amplifier (not a preamplifier) capable of driving headphones, then the iPhone level will be far too powerful compared to the instrument's output for them to ever achieve anything close to a "mix" with a passive device like this mixer. An active mixer will do exactly what you're trying to do, but make sure any such mixer is equipped with a preamp suitable for your uke's pickup or you may not be able to get enough volume from the instrument in your mix.

Is there a headphone monitoring port?

No, there is no headphone output, but there are two line outputs (1/4" and 1/8") and one of them could feed a headphone amplifier. Technically speaking, the MX41b is a passive mixer with a -12dB insertion loss, so it is unlikely it could directly drive a modern headset. Some additional amplification would be needed. No, You will need an amp to do this, regular computer speakers will not be load enough to be functional
