The only downsides are the zips being exposed so that can be uncomfortable if you lay with your arm against it on the outside. It is also really large. Obviously you need this to be comfy but it does take up a lot of space in the bed. When you get the pillow, some of the stuffing might be bunched in one area but it's really easy to unzip and adjust to your preferences. He said the shape helped support his back where he needed it and the feel of the pillow case was soft and comfortable. He has been laying the pillow on the floor for an hour or so during the day to allow it to 'fluff up' and then repositioning it on a lounger chair when he needs to lay down to ease his pain.
