It makes a vibrating sound. **Q: does it come with case?** A: Comes with a hard plastic grey case and a battery as well as 2 tips, large and small. Everything inside the case fits in a foam inset. **Q: What does this do $100 better than the half dozen others that look identical that are $100 less? What did you get for your money?** A: I bought this product and they sent me a knock off. I sent it back and they said it didn't make it there. They would not refund my money or send the correct product. I worked with this company but had to chalk it up to never buying from a overseas business. They even blocked me from leaving a review. **Q: does it help finding point on hands and feet? how exactly?** A: Sorry to take so long to respond. I'm not sure how to describe the sensor. My chiropractor introduced the device. All I know is it made a big difference that appointment and later on, here in my home after I purchased. Most points respond quite well. Some spots will react more than others. You'll know right away. Easy to hear and see exactly where the problem lies. Works for me. I have nerve damage on right leg. When cramps first start, I get the pointer. Relaxes the muscles but painful nerves are immune from painkillers. Any relaxation is greatly welcomed.
