There was no ribbon on mine as far as I remember.
If you have no strap button other than the one on the bottom of the instrument you can use a black shoe or boot lace to attach it. Or string.
Have a quick look online. Loads of videos.

Would this strap fit a Guitar Hero controller and would it be long enough for say, a young adult-sized person?

Depends on your controller, Neil... but if it's a Hero guitar with "button-type pins" to hook onto, then yes it will work.  The strap also comes with  a "shoelace" attachment should you not have a button at the top of the guitar body (in which case you can tie the strap to the neck of the guitar).  It also has sufficient flex to adjust the strap for a young adult player.

Why is there a minimum order quantity of 2, why cant I buy just one of these?

There are many other sellers on this listing aside from Amazon from whom you can buy one strap. Simply click "New (10) " and you will be shown the other sellers who also stock the item. This is the same on all Amazon listings as there are often many sellers including Amazon competing on each item. 
Every seller will have different prices and shipping rates but only one is shown as the winning seller in the main 'buy box' with 2 or 3 others shown on the bottom right of the page.
Amazon usually places Amazon first in the 'buy box' and for this particular listing they stipulate if you buy from Amazon then you must buy 2 straps instead of one. To buy one strap, select another seller and you should be able to do so without a problem.
